Eight children need current lead screening tests. One child needs an annual updated enrollment form. Three children need current medical exams. One child needs medical emergency form competed by parent. One child needs parent to completed anticipated days/hours of care and developmental history is incomomplete. The written parent permission forms must have the educator and the asssitants listed by name.

The educator and all assistants need to renew or make active the PQ Regisrty and add current training certificates. The educator needs to have a copy of their PQ Registry certificates.
Need to make the discussed changes to the parent handbook, upload it in the document library and share with parents. All current assistants need to have name and license information added to the parent handbook.
Need to complete the family child care assistant's records for all three assistants. Need medical form and MMR for the regular assistant.
The children went outside. Three children went outside the fenced in play space and climbed upon on the trampoline and one child got on a swing set that is not in the fenced in play space.
The hot water temp is reading 124.2 degrees, it must not exceed 120 degrees.
This follow-up visit inspection recorded no violations or advisories.
Need to post an exit sign on the child care room exit. The exits are accessible and safe.
An infant [redacted] was not under direct supervision while sleeping in a separate room. The infant was sleeping on the back with nothing in the playpen as required. A letter need to be completed with the corrective action and signed by the parent and a copy submitted to EEC. The staff need to complete the infant safe sleep training and submit a copy of the certificate to EEC. The licensor will conduct a follow up visit for safe sleep compliance with the infant.

Please submit a written plan for staying on top of progress reports for the children of different ages. Progress reports are up to date as of March 26, 2025 but no previous progress reports has been completed since the last visit.
Inappropriate supervision of an infant under six months while sleeping. There was appropriate supervision of the other children in care.
There are cleaning products accessible in the bathroom. The drawers on the bathroom sink has items that needs to be inaccessible to children. Need a secured gate across the open closet near the front door with supplies. Need a refrigerator thermometer should read between 32-40 degrees. Hot water temp is reading 83 degrees. The first aid it is completed.
This monitoring visit inspection recorded no violations or advisories.

The last documented evacuation drill conducted was January, 2021. Evacuation drills must be conducted once a month using alternative exits and documented.
There are cleaning products accessible to the children in the bathroom tub.
The assistant working in the program must have the name, date and time documented. There was no name listed the assistant and the day of the visit the person present was not approved.
Good judgement was not used when having person working with the children who has not be approved by EEC.

Need a gauze roll for the first aid kit.
There is a outlet in the front hall that needs to be covered. There is a cord for the computer that is running from the front hall closet and along the floor into the bedroom, the cord is a tripping hazard, needs to be made secure and safe.
The provider and assistant need to update their PQR with current training certificates and courses.
Seven children were outside with an person who is not approved to work in the program.

There must be specific places listed for taking the children off the premises. Need to submit the children's records checklist completed with all the children information that is currently enrolled.
