The program did not conduct a regular staff meeting in December 2025. Please ensure to document and conduct regular staff meetings each month.

The program did not have record of current observation of educators for some of the teachers. Please ensure to maintain accurate and complete staff records.
The personal belongings of some of the staff were within reach of children. Please ensure all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags, and purses are locked or inaccessible to children.
The program did not practice evacuation drills in some of the months. Please ensure to practice evacuation drills at least monthly.
