1. The use of a safety device or covering that prevents access to the receptacle openings was not safely installed in two electrical outlets of a {REDACTED} classroom. 2. Two electrical cords longer than six inches were not safely arranged in a {REDACTED} classroom.

At the time of the visit, the staff records checklist did not contain information related to a staff member working in the program since January 6, 2025.
Digital thermometers located inside two refrigerators were not properly working as these did not verify the temperature.

A {REDACTED} age child was not assigned to the appropriate age group as they had been assigned to an {REDACTED} fixed classroom without a development placement form on file.
A sleeping mat stored inside an {REDACTED} crib reflected a large rip, approximately 12-15 inches in length.
1. A section of the 6 FT solid panel wood fence is detached from the chain link gate entrance. The solid wood fencing reflects to be in poor conditions, with gaps in between panels and a screw was exposed on a low corner of the fence. 2. At the time of the visit, EEC found three 9-inch nails on the mulch surfacing, along with a metal hinge attached. Additional technical assistance was provided related to outdoor sensory tables.
A review of the program's emergency evacuation practice fire drill log did not contain documentation of a December 2023, and January 2024 practice drill.
A {REDACTED} age child did not wash their hands prior having their morning snack.
This investigation inspection recorded no violations or advisories.
Arrival time was not properly documented {REDACTED} room. The daily attendance reflected arrival time for six out of seven children present via the program's daily attendance APP. The program's paper copy attendance did not reflect any of the seven children signed in for the day.

1. At the time of the visit, proper handwashing procedures were not followed in a {REDACTED} classroom as children were redirected from free choice activities to the snack table without washing their hands. 2. In a second {REDACTED} classroom, staff and children did not follow proper handwashing procedures following a diaper change as an educator was observed to have placed the child back on the floor to return back to play and then proceeded to continue on their daily tasks without properly washing their own hands and or the child's hands.

A staff schedule posted on site dating 12/8/2022 was outdated as it reflected staff no longer working at the program and did not reflect new hires currently working in the classroom.
An educator did not maintain at least one hand on the child while the child was being changed on an elevated surface as it was observed a staff to have briefly two to three feet away from the diaper changing table to throw away a diaper and then return back to continue diaper changing procedures.
Two personal items intended for individual use by children, (sippy cup bottles) were not labeled with the child's name in a {REDACTED} age classroom.
At the time of the visit, an educator was observed opening children's individual food packages and hand served (no gloves were used) children's snacks (ex: cut up apples slices, fruits and nutri-grain bars, etc.) directly on to the table (not using plates and or napkins).
A review of the emergency evacuation observations for FY 2022 did not reflect an exit route used.
A child's {REDACTED} medication stored on site reflected expiration date of January 2023.
A child's Individual Health Care Plan dating 5/26/2022 indicated the child having {REDACTED}, and the need for {REDACTED} medication, however the {REDACTED} medication was not readily available at the time of the visit and the program reported not being unaware of the child being diagnosed with a chronic medication condition.

Staff monitor environment for hazards-Hazardous items not accessible to children
One child's record included an outdated Developmental History and First Aid/Emergency Consent forms which expired 9/4/2021. A second child's file did not include documentation of a {REDACTED}. Progress reports did not include completion dates.
One staff record did not include documentation of {REDACTED}, two references and of an interview. Four staff records did not include documentation of an orientation and bi-monthly observations conducted.
Monitoring of the environment was not conducted as hazardous items (2 small containers of CleanCide Wipes-indicating Caution- keep out of reach of children) were located on the floor next to the water closet of a preschool bathroom, accessible to children. Items were removed upon Licensor's request. Provide a plan for how staff will monitor the environment for hazards/hazardous materials.
