Licensor observed that CHILD1 was missing the following: physical description, receipt of parent handbook, visit notice, parent signature/date on page 5, permission to transport child to a medical facility and perform CPR/FA was not signed, topical medication was not filled out. Emergency card was not filled out. CHILD 2: was missing admission date, receipt of parent handbook, visit notice, permission to transport child to medical facility and perform CPR/FA was missing educator and assistant’s name. CHILD 3: missing physical description, missing child’s name on page 2, receipt of parent handbook missing parent signature/date. Permission to go to the park was not signed/dated, permission to transport to medical facility and perform CPR/FA was not filled out. Topical Medication was not listed. Emergency card was not filled out.
