The licensor reviewed 5 of the children records. The following were observed: 1.Three of the children records are not updated. The parent signatures are due to renew on 7/15/25 6/25/25, and 5/15/25. 2.Two of the children records did not list regular medications that is used by the children that are EpiPen and Diazepam (for ashtma)(see page 4 on the enrollment packet). 3.Five of the children record did not list the assistants names on the general permission to basic transportation or permission to transport to medical facility, see page 6 on the enrollment packet). 4.One the children record did not have an updated physical. The physical was due 7/17/25. 5.Two of the children did not have evidence of a physical. 6.One of the children did not have the individual health care plan in the child’s record. 7.One of the children record did not have a picture or physical description of the child. 8.One of the children record is not complete when the individual health care plan did not have the authorization of a licensed health care practitioner.
