There was an infant sleeping on a swing, with a blanket laid on top of them. Please be aware that all infants under the age of 12 months must be put to sleep on their stomachs, on a flat surface, with no loose items located near them (i.e.: blankets, toys, bibs). Within seven days of this visit, please return to EEC the Safe Sleep pledge, Safe Sleep Parent Letter, and certificate of Safe Sleep Training from Strong Start.

Supervision
There were children napping on the first level and second level of the home. Licensor discussed the regulation requiring that Educator remain on the same floor level as children at all times. Please add new sleeping arrangement to ensure all children remain on the same floor level.

Educator gave Licensor a virtual tour of the home and there was only one hazard observed. There was an exercise bike in one of the bedrooms. Please remove this or make it inaccessible to childcare children. Upload a picture once completed.

Educator sent Licensor the Children's Record Checklist for three children. All [REDACTED]physical examinations and progress reports were out of date. Please ensure these records are updated for all records and confirm once completed. Please also include your plan on how to maintain records up to date in the future.
There are two areas of fencing that are not stable. Please add posts to the fencing to ensure it is sturdy and stable. There were no gates at the bottom of the deck stairs. Please add gates to ensure children don't have access to the stairs. Please upload pictures of fencing and gates once completed.
Educator's emergency preparedness plan was incomplete. Please ensure the emergency written plan details procedures for meeting potential emergencies including but not limited to missing children, the evacuation of children from the program in the event of a fire, natural disaster, loss of power, heat or hot water or other emergency situation.
Educator does not participate in tooth brushing and does not have oral non-participation forms on file for each child. Licensor discussed with Educator that they must assist children in brushing their teeth whenever they are in care for more than four hours or whenever they consume a meal while in care.

Licensor walked through licensed space and observed the following hazards: Upper Level: **KITCHEN**- One drawer had loose cable and scissors accessible. Please make these items inaccessible. **DINING ROOM**- no hazards **LIVING ROOM**- no hazards. **BEDROOM 1**- no hazards. **BEDROOM 2**- no hazards. **BATHROOM**- no hazards. Lower Level: **PLAYROOM**- This room was cluttered with toys and other items. Educator is currently using this room as a storage area. Educator would like to move her playroom to the room adjacent to her current playroom. Licensor discussed with Educator that she must start a new transaction for a change of space. Please start this transaction or clean out current playroom.
