First aid kit was missing children's emergency cards. Please take forms from children's enrollment packet and add to first aid kit.
There is one child enrolled that has medications and requires an epi-pen. There was no IHCP in the record. Please obtain IHCP from doctor and place into read.; confirm once completed.
There is one child enrolled that has allergies and is prescribed life saving medication. This was not posted. Please ensure this is posted in a confidential manner; confirm once completed.
Educator provided children with snacks and they moved freely while eating. Also, the children did not wash hands beforehand. Please ensure children are seated when eating/drinking to avoid a choking hazard. Furthermore, ensure all hands are washed prior to eating; outline how this will be done moving forward.
This follow-up visit inspection recorded no violations or advisories.
There was a seven month old child napping with a blanket. Per Safe Sleep regulations, infants under 12 months should not be put to sleep with any loose items in their area. Licensor provided Educator with Safe Sleep Pledge, Safe Sleep Parent Letter, the policy, and information on the safe sleep training. Please get letters back to Licensor and take training within one week, and confirm with Licensor once everything is completed.

Licensor reviewed children's records and some progress reports were out of date. Please update all progress reports and confirm once completed.
There were napping children in each of the bedrooms, and the doors were not ajar and lacked adequate lighting. Please provide napping procedure that will be used moving forward that ensures appropriate supervision is ensured.
Licensor walked through indoor space and the following hazards were observed: Basement: The main play room was under construction and there were no barriers blocking access. Please add gates to ensure children don't have access, and upload pictures once completed. There were accessible electrical outlets in bedroom; ensure all outlets are covered and confirm once completed. Please ensure bedrooms are clean and free of hazards; upload pictures once completed. There was a box of cleaning products accessible in the kitchen; make these items inaccessible and confirm once completed. Licensor walked through outdoor space (fenced in back yard) and there were no hazards observed.
Educator doesn’t have sufficient ongoing professional development hours and has not met the diverse learner’s requirement. Please take additional trainings to meet the 30 hours and ensure that the trainings are on diverse topics related to caring for children with different needs. Please email Licensor with certificates.

Outdoor Playspace - Hazards
There is a new deck. Licensor observed the following hazards: There was an uncovered grill accessible. Educator was also using furniture (such as the grill and bench) as barriers to stairs. There were no gates at the bottom of the deck stairs. The top of the stairs will require fixed gates. Please resolve all of these hazards and upload pictures once completed.

Licensor reviewed three children’s records and one was missing a physical examination form. Please obtain this and add to record. Ensure all records are all up to date.
Exits were clear and safe. Some of the exits were missing signage. Please add these and upload pictures once completed.

Licensor walked through the indoor licensed space and the following hazards were observed: There was an elliptical in the basement area. Please remove from the space as exercise equipment cannot be accessible to children. In the dining area, there were unsecured shelving. Please anchor these to avoid a tipping hazard. There is an uncovered trash can in the restroom. Please remove and ensure all trash cans in the licensed space are covered. Please confirm once all hazards are resolved.
There was a power strip located in one of the bedrooms. Please ensure all power strips and electrical outlets are covered.
There is chipping/peeling paint on the front door frame. Please resolve this and confirm once completed; a follow up visit will be completed.
Licensor walked through the outdoor space and observed the following hazards: Educator lives on a busy street and once had fence built; she reports that the fence was taken down while she built a new deck. Please reinstall a fence and confirm once completed. A follow up visit will be completed. There was also an uncovered fire pit, lawn machinery/equipment accessible, and the shed was open. Please ensure all of these hazards are made inaccessible to childcare children.
