Staff Records
Six child records were reviewed during the visit and found one child's record is missing a doctor's action plan for a chronic medical condition.
The girls' bathroom in the half day program was observed to have a plunger sitting next to the toilet. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children.

Emergency Evacuation
Three IHCPs were incomplete. Two were missing the names of the educators trained to administer treatment and the third was missing medical treatment and potential consequences if treatment is not provided. A fourth child’s record was missing an IHCP for a chronic medical condition.

Two staff records were reviewed and found both staff records did not have full compliance with evidence of immunity to MMR (one staff had 1 date and a titer for Rubella and Mumps but not Measles) and the second staff record only had one vaccination date. These staff records were also missing annual evaluations (one record had the last annual as 2018 and the second staff has worked for one year) and bi-monthly observations.
BRC Navigator was reviewed on 11/17/2025 and found that two active staff have expired BRCs.
The Evacuation Drill Log was observed during the visit and found a drill was not documented in June 2025.

Three staff records were reviewed and found: One staff record was missing the date of hire, orientation checklist, bi-monthly observations, and one MMR vaccine; One staff record was missing the date of hire, annual PQ registration, evidence of an interview, references, orientation checklist, and bi-monthly observations; One staff record is missing a physical stating the educator is in good health, one reference check, and bi-monthlies observations.
Licensor observed the 45 minute block of rotating centers. Five classrooms of children were rotating between all five classrooms to do activities such as science, art and motor. During this time, attendance was not maintained in each of the classrooms. In the event of an emergency the educators must have an attendance of what children are with them to ensure all children are accounted for.
The following was observed to be accessible to children: In the front cluster room back left corner classroom, two ceiling tiles are stained; In the front cluster room back right corner classroom, the cords on top of the block shelf were not secure; In the front cluster room front right corner classroom a box of open gloves and three baggies of wipes sat on low shelves (moved during visit); and a plunger was found in the back cluster bathroom.
The evacuation drill log was reviewed and found there was no evidence of drills conducted between September 2023 and July 2024.

There was not an accurate attendance for 4 groups of preschoolers. The program space is divided into 4 "rooms"; each assigned an educator. However, during activity time, children are able to move from one group to another, with no attendance documented. Technical assistance was provided.
One group of preschool children was not supervised as the educator left the group to help a child in the bathroom.
