The staff record review reflected the following: 1) A record with insufficient evidence of MMR immunity and ongoing bi-monthly observations. 2) A second record with no evidence of current physical, bi-monthly observations and annual evaluation. 3) A third record with insufficient evidence of ongoing bi-monthly observations.
During the visit, EEC observed multiple lamps with long electrical cords accessible to the children.
1) During the walkthrough, EEC observed snow obstructing the exit door in the preschool classroom (#69) 2) The program was not able to produce evidence of evacuation/fire drill log from January thru May 2025 and September through December 2025.
Several medications indicated on children individual health care plans were not readily available during the visit.
At the time of the visit, a child's EpiPen was expired.

The children's record review reflected the following: 1) A record contained no evidence of current physical, annual lead screening, immunization records and the FA/CPR parental consent form did not include a parent signature. 2) A record contained no evidence of current CPR/FA parental consent and transportation plan form.
EEC observed napping materials not in good repair, exposing the bedding material.
1) A child's EpiPen was not in the container in which it was originally dispensed and with the original label affixed. 2) A child's medication which is indicated on their individual health care plan was not readily available.

During the health and safety walkthrough, the EEC Licensor observed eight toddlers in a room that, based on EEC measurements, the maximum capacity allowed is six children.
A child's record did not contain a current individual health plan for their medical condition.
At the time of the visit, a child's EpiPen was expired, 12/2024.

The background record check review reflects a staff member with an expired BRC. Please ensure that all BRC renewal requests are completed prior to expiration.
At the time of the visit, the stairway of the new indoor space (#69) did not have a permanent barrier installed at the top of stairs and a pressure gate at the bottom of the stairway.
