The space was observed to have materials that made some of the areas appear cluttered. Some of the materials observed were large cardboard boxes, suitcases, a portable diapering table that was not in use, and packages that were unopened. It was also observed that materials that were not for napping were being stored in one of the pack and plays.

At time of visit licensor knocked at the front door and was able to observe educator outside. Licensor walked to the backyard where it was observed educator was walking inside with three children. Once licensor walked inside it was observed there was one child sleeping in a pack and play. Supervision was reviewed with the educator. Educator reported walking in and out to check on sleeping child. During visit it was observed a daycare child was able to access stairways to the second floor. Gate was observed to be open.

Indoor space was observed to be in disarray each room appeared unorganized. The living room was observed to have multiple adult shoes laying on the floor that cause a tripping hazard, there were rose pedals on the floor, a rubber band on the floor, there were a couple of car seats, and infant chairs that were cluttering the space in which children play. The children’s table in the playroom appeared cluttered. There was a small bookshelf in the playroom that was easily moveable and needs to be secured. In the front entrance there was access tall coat rack that was easily moveable. In the kitchen there was access to a vacuum with the electrical cord undone. It was observed that there was a slide in the backyard that measured over 18 inches tall and did not meet the fall zones and ground surfacing requirements.
