The following were observed: -5: Two screws were protruding from the metal step plate. The tape on the sand table was peeling, exposing a sticky edge. -6: A cord over 6 inches was accessible to children (in room 6 and another SA room). The wooden door had a broken piece, exposing a sharp edge. There was chipped paint on one wall. There was rust on the connector pipe to the bubbler. -One School age room had ripped carpet which created a tripping hazard.
