You could not come into the room from this entrance/ exit, without the staff inside unlocking the door. -In the infant room, the blue vinyl play mat had torn areas on the edges. -On the metal heater cover (protector), there was a quarter size hole, with sharp edges around the hole. -In the small hallway to the outdoors, through Room 3, two brooms were leaning against the wall. These create a trip hazard for anyone walking through this area. -The play kitchen in SA was tippable.

The Director was asked if staff in the rooms were qualified/ certified. She indicated: The infant teacher and the upstairs PS teacher were EEC Certified The male staff was an assistant teacher. The program had "made" the downstairs preschool staff a teacher, the same way that they had "during CoVid". (Most CoVid procedures that were initiated through EEC, ended July 2021) When asked if they had used minimum hiring requirements to qualify the staff, the Director was not familiar with the minimum hiring requirements and stated they had trained her at the program. Further conversation indicated that the staff person did not have any evidence of being qualified by minimum hiring standards or by EEC certification. At the time of the visit, there was no documentation that would support that the staff person was teacher qualified.

Upon arrival to the program around 12:15PM, the Director and two staff persons were in the Directors office (directly inside the side door). Licensing indicated they were there to do a monitoring visit. Both staff exited the office and headed toward the toddler room. The Director came out from behind the desk and preceeded licensing into the toddler room. At that time: One staff was present in this room of 9 toddlers, prior to the Director entering and joining ratio. I went directly to the I/T room and one of the staff who had been in the office upon arrival, was just walking into the room. There were 4 sleeping infants. All were verified over 12 months of age. There was no other staff present in the room. I went back into the toddler room and into the preschool room (connected to the toddler room). There was one staff person with 8 preschool children. I observed the upstairs PS room, and the staff person did not know how many children she was assigned to supervise. (The Director stated during the exit interview that this staff had just relieved another staff from break). Downstairs, the Director was then observed supervising the PS room of 8 children and toddler room of 9 children, alone, by standing between the doorway to the two rooms. A staff did come downstairs and take over the PS room. Another male staff joined the toddler room. Children were on rest mats and appeared to be resting. There were no other staff available to relieve the Director, so the exit interview was initiated at the far end of the room, near the doorway towards the office. During this time, the male staff exited the toddler room, into the kitchen, which is not part of the toddler room. The room was again out of ratio.
