1) Throughout the visit, the program had a "difficult" time remaining in ratio. At several points, there were more than 26 children and two staff in the gym space. Additionally, the Assistant Site Coordinator left the grouping in the gym without a YMCA staff member present in order to assist another educator with a transportation issue. 2) On the day of the visit, staff had 31 children total in Room 11, which measures for 26 children total, the program exceeded the total capacity allowed for that classroom, creating a potential hazard.

Throughout the entire visit, it was observed that children were not properly supervised. Educators were not clear regarding which children were in their groups or how many children they were responsible for. While walking to the bathroom, Child C heard the sound of the vacuum and after covering their ears, attempted to close themselves off into the janitor closet, but the Licensor was able to intervene. A YMCA staff did not witness that incident, as Child C was sent to the bathroom alone, without staff supervising.

As a result, children were climbing into the pickle ball courts of adult players (not related to the YMCA) and accessing the infant equipment on the playground without being redirected to use equipment that is appropriate for school aged children.
