1.The gym space reflected exposed wiring from underneath a corner opening of a wooden bench area. 2. At the time of the visit, a staff's personal belonging (Purse) was placed on top of chair, accessible to children present in the library. 3. Sanitizing wipes indicating "Keep Out of Reach of Children" were placed on top of a table, within reach of children present in the library.

1. At the time of the visit, documentation of an interview, employment application/resume, and verbal verification of two references were not available for review in two staff record files. 2. Documentation of bi-monthly staff observations were not available for review in a staff's record file.
At the time of the visit, a copy of the EEC License certificate was not posted on site.
Thermometers verifying temperatures were not maintained in a refrigerator.
Proper hand-washing procedures were not maintained as children did not wash their hands with soap and running water prior to eating snack.
