Licensor reviewed four random children’s records. One file had expired enrollment paperwork. Please ensure all children's files are updated annually.
Licensor did a walkthrough of all licensed space and found the following hazards: *Playroom: Large heater in playroom is extremely hot when touched and requires some form of barricade or cover to protect children. Please upload picture of how you plan to protect children from accessing and touching hot heater. *Kitchen: Dog food was in bowl on floor within reach of children. Please remove dog food from children's reach at all times.

Licensor walked through all indoor/outdoor space and found the following health risks: *Playroom: Tall ladder was leaned against shelving along with tool bag containing hammers and other items below. Playroom is full of stored materials and equipment making space hard for children to navigate and overstimulating. Please remove ladder/tools from licensed playroom and provide more open space for children to traverse the room by removing some of the larger items stored away inhibiting them from currently doing so.
Licensor did a walkthrough of all indoor/outdoor space and found the following hazards: *Outdoors: Chipping paint around stairs accessing family room as well as on backside of house by playroom. Please remove any chipping paint found on outside of house specifically targeting mentioned areas above.
There were several records missing children's physicals. Please do thorough review of records to ensure everything is up to date and all medical information is added; confirm once completed.
Licensor Ayala and Supervisor Faulkner walked through the licensed space and observed the following health risks: In the kitchen, there were several rotting pumpkins in the space where children had access to. One of the pumpkins was leaking liquid onto the floor and emanated a strong, foul smell. Licensor asked Educator to remove all rotting pumpkins from the home. In the play room, there were light bulbs accessible. Please remove all light bulbs from children's access. In the hallway, there was a plastic bags full of other plastic bags. Supervisor Faulkner removed these bags from the accessible area. The locks under the kitchen sink were broken, therefore cleaning products were accessible. Please replace lock or make cleaning products inaccessible. Please confirm once all items are completed.

Educator and assistant both diapered children during this visit. Educator left a soiled diaper on the floor after diapering a child, which other children had access to. Please ensure proper diapering procedures are completed (thoroughly review EEC's policy). Please outline what the diapering and toileting procedure will be moving forward.
Staff and children did not wash hands following diapering. Educator reported that she wipes the children's hands with wipes. Please ensure hands are washed following each diapering change/toileting in the bathroom sink, with soap and water.
There continues to be chipping paint on the exterior of the home. Though the chipping paint located in the front side of the home has been resolved, there is now chipping paint on the back of the home near the pool. Please ensure all chipping/peeling paint is completely removed from the home. Furthermore, ensure there is immediate action to any further chipping or peeling paint. Confirm once completed and a follow up visit will be scheduled.
Licensor Ayala and Supervisor Dennis walked through the licensed space and observed the following hazards: The home appeared overall unorganized and unkempt. There were items sporadically stored, thus making the space look cluttered. In the kitchen, there is a door which leads to the above ground pool. This door was partially unlocked. Educator reports that the lock broke recently. Please fix this lock and add an additional lock which children can't reach. Please upload pictures once completed. Additionally, there was dog food accessible to children. Please ensure all dog food and water is not accessible to children. Licensor Ayala and Supervisor Faulkner walked through the outdoor space and observed numerous hazards, which were as follows: There were two sheds that were completely open. These sheds contained hazards such as lawn equipment, exposed machine blades, cleaning products, tools, etc. There was also a bag of electrical wires in the middle of the yard. Please remove wires from the space. Furthermore, here was access to the underside of the deck, which appeared unsafe for children. Please add barrier to ensure children can't access this area.

Licensor reviewed records and there were records missing physical exams and annual update forms. Please update all records and confirm once completed.
Licensor walked through the indoor, licensed area and the following hazards were observed: The play room flooded and Educator is in the process of cleaning it. There are toys and items strewn about and the area is not accessible. Please clean this room and ensure all hazards are removed. Please upload pictures once completed. The table top and counter tops were cluttered with many items, which the children have access to. Please ensure table/counter tops are free of clutter to ensure the area is clean and free of hazards; please upload pictures once completed.
There was chipping/peeling paint on different areas of the home (front stairwell and sides of home). Please ensure all chipping/peeling paint is removed and confirm once completed; a follow up visit will be scheduled at that time.

This monitoring visit inspection recorded no violations or advisories.

There was a power strip located in the dining room that was accessible to the children. Please ensure this is always made inaccessible to childcare children.
Licensor walked through licensed space and observed the following hazards: **Living room**- There were scissors accessible to children; please ensure sharp objects are always kept inaccessible to childcare children. There was also exercise equipment located within the living room. Please ensure there is no exercise equipment kept within licensed space. **Dining room**- There was a power strip accessible. **Kitchen**- No hazards. **Bathroom**- No hazards. **Playroom**- No hazards.
Educator does not keep medical letter or attendance for her assistant (whom is her husband). Please ensure staff records are maintained at all times.
There were excessive outdoor hazards, many of which have been addressed in prior visits: They are as follows: - There was a long power cord that was connected to a power strip, which had 3 empty outlets. - This power cord was coming from a leaf blower, which was accessible. - On top of the shed located by the play area, there was construction material about to tip off of the top of the shed- which could fall onto a child. - There were loose wires accessible. - There was a sand pit without the proper cover; the tarp was sitting right beside the sand pit. - There are two climbing domes located in the outdoor play area, which are prohibited. - Educator uses a fenced in area for the younger children; this is fenced with a chicken wire type of fence. This fence is unstable and falling over. Please fix this fence to ensure it is stable. - There is an unused hot tub turned over. Educator reports that a company is coming to pick this up, but is currently accessible. - Many of toys and equipment located in the outdoor play space are unstable, old, and dirty. - Throughout the entirety of the yard, there are many random objects that include tires, bins, old equipment, construction materials, etc. For these mentioned hazards, that have been of concern throughout several visits, this Licensor is referring this Educator to coaching. The hope is that with this coaching, Educator will have guidance on how to eliminate and reduce the amount of hazards in the outdoor space.

There is a small deck located off of living room. The space in between railing is 6 inches wide. Please ensure this space does not exceed 3.5 inches.
Educator has an excessive amounts of play equipment and structures. Educator has two climbing domes which are prohibited by EEC. Please remove these domes from the play areas.
CPR expiration date 8/17/19. First aid expiration date 8/16/21. Educator reports that she just completed another CPR course and could not find the certification at the time of this visit. Once located, upload the certification. If certificate is not located, please take another course within 7 days and upload certification.

The playroom exit was obstructed by toys and other items. Educator did move these items during the visit. Please ensure these are clear at all times.
Licensor walked through licensed space and observed the following hazards: INDOOR- Playroom- This room was very cluttered with toys and play equipment. There were men’s body care bottles and batteries accessible. The heat was also turned off in this room. Please ensure this room is free of clutter and any hazards. Family Room- There was a ladder and step stool located in living room that were over 4 feet tall, which poses a tipping hazard. Please remove these items from the living room to avoid injuries. Kitchen- The counters were covered in food, papers and other items. There was cat and dog food on the floor. The child proof lock under the sink was broken and cleaning products were accessible. There was a large trash bag on the floor and not in a proper closed container. Please address all of these hazards and make them all inaccessible. Dining Room- No hazards. Bathroom- The child proof locks on drawers need to be replaced. There was a blow dryer and hair iron located in these drawers, with loose cables. There was medication also located on kitchen sink counter. Please make changes to resolve these items. OUTDOORS- There were trash bags located by front door not placed in a closed container. Please ensure all trash are in covered containers. There was a lawn mower accessible on the property. Please ensure lawn equipment is covered or stored securely. There was a ladder placed on the side of the home that is over 4ft tall. Please remove this ladder during childcare hours to avoid injury. The shed located in the backyard was open with a snow blower, wires, tools, gasoline canisters, and wood accessible. Ensure this shed is locked during childcare hours. There were 2 climbing domes in the play area, which are prohibited. Please remove all climbing domes from play area. The crawl space under the porch was accessible as the trellis was falling over. Please secure this trellis to ensure children cannot climb inside. There were several bins stacked higher than 4 ft which poses a tipping hazard. Please address all of these hazards and ensure that outdoor play space is safe, clean, and appropriate for children.
