Th following was observed and accessible to children: The bolts on each gate were long and a potential entanglement hazard, the wooden shed door was splintered at the bottom, and there was rust and chipped paint on the climber poles and swing set poles.

It was observed that the vinyl baseboard was peeling in one of the stalls in BATHROOM #2.

review of nine Individual Health Care Plans indicated that five did not identify which staff are designated and trained to administer that child's medication.

The following was observed and accessible to children: -In ROOM 7, the sensory table rug had curled edges, a potential trip hazard. -In the GIRLS BATHROOM, there was rust stains near the soap dispenser.
It was observed that there was chipped paint in ROOMS 3B and 7.

The following was observed to be accessible to children, in: Gross Motor Area:- one outlet on the left hand side of the room not covered. PRESCHOOL CLASSROOM 7: -A piece of the electrical outlet cover broken on the adjacent wall to the right of the outside door.
Indoor Space
The following was observed in the outdoor space accessible to children: -The paint on top of the chain link fence is peeling off to the left of the shed, and across the back of the left side playground. -Pieces along the bottom of the fence on the far left side are broken and have a sharp edge. -Exposed mulch paper poses as a tripping hazard on the right side of the shed, in the trench area, and to the left of the second door leading inside the building from the playground space. -A floor mat laying on the mulch poses as a tripping hazard. -Two manhole covers pose as a tripping hazard.
Upon reviewing the attendance log in classroom 3B it was observed to be inaccurate. The Educators had recorded five children present when there were six children.
Outdoor Space - R Apr 2025
The following was observed to be accessible to children in classroom: 3B: - Tape residue on the wooden child-sized chairs and the table. PS ROOM 1: -A broom was leaning against the wall but made inaccessible during the visit. -Nail holes and a nail protruding out of the window sill along the bottom left hand corner. -Tape residue on the cover of the sand table. Motor Space: -Stacked and tippable child items. CLASSROOM 6: -Tape residue on the sand table cover and on the literacy center table. -Two pairs of adult scissors on a low shelf in the cabinet. -A pocketbook on a low shelf. -A loose extension cord along the back wall in the calendar area. -A loose hanging phone cord. -Rust along the bottom of the partition in the children's bathroom. CLASSROOM 7 (PRESCHOOL): -A white drawer units with orange and green bins not secured and tippable. -A wooden bookshelf leaning against the sleep cots not secured and tippable. -One window blind cord hanging low longer than six inches. This was fixed during the visit. -Tape residue on the bookshelf in the calendar area. CLASSROOM 8: -Tape residue on top of the bookshelf under the phone.
Toxic Substances
The following was observed to be accessible to children in classroom 3B: A spray cleaning bottle at the edge of the cabinet. This bottle was moved during the visit.
The consent form for a child's nonprescriptive medication (Buttpaste) signed by the parent was not dated.
The following was observed to be accessible to children, in: CLASSROOM 3B: - Peeled paint in on the wall in the dramatic kitchen space. PRESCHOOL CLASSROOM 7: -Chipping paint on the wall directly below the class rules posters.

There was food waste in two trash cans that were not covered.
Outdoors there was rust and chipping paint on the fence/ gate. The mesh divider was sagging and was not firmly secured to the posts.
