After reviewing three Staff Records the following items were observed to be missing: -Bi-monthly observations for all three staff records. None have been completed this school year.
The following was observed to be accessible to children: -An area of the side fence to the right of the near deck structure has been bent and is tipping over into the playground space. Children were observed to be playing on the tires within six feet of the bent fence. Upon Licensor's request, the children were not allowed to play on the tires within six feet of the broken fence. -The vertical rungs on the black gate separating the outdoor space and the newly built outdoor classroom space measured at approximately 4 inches across. An entrapment is defined as "Any opening that is greater than 3½ inches but less than 9 inches that can entrap a child’s head, creating a risk for serious injury or death.”
After reviewing six Child Records the following items were observed: -Two children's Individual Health Care Plans were not current (expired 9/2022). -One child's Individual Health Care Plan was not dated and was missing the following information: potential side effects of treatment, staff trained to administer, and who trained the staff.
The following was observed to be accessible to children in the classrooms: -In GREEN HARBOR, a black teacher's chair was obstructing the side egress. The chair was relocated during the visit by the Director. -Wooden shelving units and play kitchen sets throughout the program were not secured and tippable. -Loose speaker cords longer than six inches were observed in BLUE FISH, GREEN HARBOR and SUNRISE. -The bathroom door connected to SUNRISE is breaking at the bottom and wood is starting to splinter off.
Medication
Individual Health Care Plan (IHCP) - R Apr 2025
The children’s checklist completed by the program indicated: -Four files did not have a required lead screening. -Three files had no evidence of a physical exam. -Six files had out dated physicals.
A review of four children's files indicated that four Individual Health Care Plans (IHCP) were incomplete.
The program did not have a written parental authorization form for (REDACTED) stored at the program for a specific child who is enrolled.

This change of space visit inspection recorded no violations or advisories.
The program did not have a medication consent for a child that had a (REDACTED).
