EEC Assistant
The educator had an assistant whose license expired in April 2023 working in her child care until September 7th 2023.
The Educator continued to use an assistant whose certificate expired in April 2023 to care for children and count in her ratio from April -September. There were multiple days a weed during that time period that the educator was over enrolled since she did not have an approved assistant working with her
The educator is not keeping staff records

The attendance records were not filled on for the day of the visit. A review previous days showed that the times of arrival and departure were filled in on some days, but it was not consistent. This is an issue that has been addressed with the Educator previously.
There were 9 children with the Educator. There was another adult on the premises, however, this person has not been approved by EEC to work as an assistant. This has been an issue several times in the past. One child did leave during the visit, around 11 am.
Progress Reports
There updated medical forms and Annual Update forms missing form children’s files.
The Educator stated that she has asked for the IHCP, but has not received it yet. She said that the parent told her that the (REDACTED) is no longer a concern and will bring medical documentation stating such.
There is no proof of an Emergency Preparedness Plan.
The Emergency Cards were not with the first aid kit.
There was a person working as an assistant, however this person had not yet been approved by EEC. The Educator reported that her approved assistant called in sick on the day of the visit. This is an issue that has been discussed with the Educator before.

There are 18 children enrolled in the program. Ten children’s records were reviewed. Four were missing the signature for the acknowledgment of the Parent Handbook on page 3 of the enrollment packet. Five were missing current medical documentation. Three needed Annual Update forms.

There was a person working as an assistant, however this person had not been approved by EEC. The Educator reported that her approved assistant recently quit without notice.
