At the time of the visit four children with emergency medications were reviewed and the following was found: one child was missing a portion of their prescribed medication. All emergency medications must be readily available at all times.

At the time of the visit four staff files were reviewed and the following was found: one file was missing a current physical, and one file was missing the staff information form, evidence of MMR and completed orientation form.

At the time of the visit four child files were reviewed and the following was found: one child's file needed the developmental history, transportation form, consents, and emergency form updated. The above listed items need updated annually.
At the time of the visit there were two areas of peeling paint in the Adventurers II classroom book area. There were also exhaust vents in Discoverers II and Adventurers I that were beginning to rust and were in need of repainting.
There were no evacuation drills logged for January or February 2025. Drills should be conducted and logged on a monthly basis.

At the time of the visit two children with emergency medical conditions were reviewed and the following was found: one child did not have an individual health care plan for their condition.

Several pass through doorways between classrooms did not contain appropriate evacuations postings.

At the time of the visit three staff files were reviewed and the following was found: Staff 1: had no orientation form Staff 2: had no orientation form, verification of reference, hire date, or annual review Staff 3: had no references or annual review
At the time of the visit attendance was not correct.
There were only four evacuation drills conducted over the last year. These were drills conducted with the fire department on their inspection visit days. Evacuation drills should occur on a monthly basis.
At the time of the visit three children requiring individual health care plans were reviewed and the following was found: one child did not have the necessary IHCP form, and two were missing EEC required information.

The following was observed and accessible to children: Infant 1 Classroom -There were five cords longer than six inches hanging from the windows. Infant II Classroom -The heat vent was dusty. Pre-Kindergarten Classroom -The heat vent was dusty. Preschool Bathrooms -Three paper towel dispensers had rust. -Three trash cans did not have covers. One of the trash cans had plastic gloves. Outdoor Hallway -A rake and broom were leaning against the wall.
A review of four children's records indicated: -Three records did not have a completed Individual Health Care Plan (IHCP). -One child's record indicated that they had (REDACTED) however, the record did not have an IHCP.
A child attending the program required an (REDACTED) according to their current physical examination. The program did not have a medication for this child. As a result, a current (REDACTED) was not available for a child who might need it in an emergency situation.

In Achievers 2, a large television was tippable. In two rooms, the program used a slide that was converted from a crib. The slide did not have specifications or safey measures to protect children from falling from the side.

A teacher was observed cleaning the tables while children were sitting at the tables.
