A review of 15 children’s records showed that: 6 general permission were either not completed or signed but blank. One child need the emergency permission to transport to hospital signed
The first aid kit did not contain the emergency contacts cards for the children currently enrolled
The second exit was not clear today, you could not use it due to the snow not being cleared. The only exit that was clear was the side door.

A review of the children’s records show that 1 child pg. 2 was blank, and a general permission was blank
Although the educator is documenting monthly fire drills they are not being done correctly.

The infant was not given time to explore on her own, she was held by the educator and then given a toy to play with while on the educators lap and then placed in a high chair for lunch

When the licensor knocked on the door to start the visit. A (REDACTED) daycare child answered the door. The educator was in the play room sitting in a chair, with a child asleep on her. She initially called out, can I help you and then came to the door when the licensor identified herself

The children’s Emergency Cards were not with the first aid kit. The Educator stated that she had copies of them, but she could not find where she put them.

The infant was placed in a bouncy seat on the floor while the Educator went into the bathroom to change diapers.
The Educator did not have proof of an evacuation drill for December 2019. Additionally, she is not recording the time that it takes to complete an evacuation drill.

Records for the eight children present were reviewed. One child did not have an enrollment packet, three children had incomplete/non-specific General Permission sections of the enrollment packet, and five children needed Annual Update forms.
Progress reports were reviewed for the 8 children present. Two of the children needed proof of current progress reports.
The first aid kit was missing instant cold packs and emergency cards for the children who are currently enrolled in the program.
The Educator did not have proof of a written emergency preparedness plan.
The Educator fed a bottle to one of the infants while the infant was sitting in a bouncy seat.
The one toothbrush was stored in a bag in the closet. Discussed that toothbrushes must be stored open to the air and individually.
The Educator reports that most of the children do not brush their teeth during during child care hours. None of the parents have signed the Oral Health Non-participation form.
There were 8 children present with the Educator on the day of the visit. Only one of the children was school aged. A review of the attendance records showed that the Educator was also out of ratio on January 13, 2020, November 25, 2019, November 18, 2019, September 30, 2019. This is an issue that has been addressed with the Educator several times in the past.
