The licensor reviewed a sample packet of children records and observed the following: 1)Child #4 is missing lead screening results.
The licensor observed the following indoor/outdoor hazards: 1)The water temperature was 125*F. 2)The outdoor fence was missing the half of the gate. 3)multiple bottles of motor oil were observed on the ground and accessible to children. 4)Several loose wood panels were accessible to children and poses a tripping hazard. 5)A mini grill was observed uncovered. 6)A bucket filled with water was accessible. 7)Multiple tires were scattered throughout the backyard. 8)A cage leaning against the wall poses a tipping hazard. 9)An unraveled hose poses a tripping hazard. 10)A long rope with a tire attached at the end was accessible posing a strangulation hazard.
The educator did not have a well-established routine in place. The educator stated that due to being closed for the previous couple of weeks, she has not planned any activities. When asked, the educator was unable to show planned children's activities for the month of February either.

The licensor reviewed the educator's attendance log and observed it to be missing information. For 2/23-2/25 the educator had the children’s names and no arrival time or departure time was observed to be noted.
