Observations from original visit on 2020-01-27: Licensor observed a {REDACTED} laying on his back on the educator’s bed on top of a fluffy blanket. The {REDACTED} was awake and was crying loudly. The educator is not following the Safe Sleep Practices. This Licensor cannot confirm the length of time that this {REDACTED} was left unattended in the educator's bedroom. This item was previously cited on 10/3/2018. Today's observation constitutes the second Safe Sleep violation. Corrective Plan received on: N/A Observations from follow up visit on 2/4/2020: The educator reported that the {REDACTED} observed on 1/27/2020 is no longer enrolled in the program. There is a {REDACTED} enrolled in the program. Licensor observed 2 open playpens in the living room, one pink and one gray. The educator reported that they are both open because she hasn’t had the time to close and remove the pink playpen. This Licensor asked the educator which playpen is used by the {REDACTED} enrolled in the program. The educator pointed to the gray playpen and reported that it is the one she uses for the {REDACTED} to sleep. This playpen was closed and inside of its bag on 1/27/2020. Licensor observed a small mattress underneath the mattress in the gray playpen. The educator is failing to provide a safe sleep environment for {REDACTED} months.
