The program has recently had a change of administrator, and they did noy notify EEC of the Administrative change.
On the day of the visit the program had not documented the arrival times for up to 20 children that were present.

Building Inspection and Fire Inspection
On the day of the visit the program’s Building Inspection certificate had an expiration date of 3/4/25. The Fire Inspection had an expiration date of 7/16/23.
Administration
Review of the staff records checklist revealed that 3 out of 4 Educators did not have evidence of a physical exam.
During the visit children were observed using hand sanitizing gel in place of proper handwashing.
Licensor was unable to assess compliance with staff meeting because the documentation did not include the time the meetings started or the time the meetings ended.

A random review of staff files revealed that at least two files did not contain evidence orientation, PQR, or staff forms. One file for a Group Leader did not include evidence of qualifications for their position.
Review of children’s records revealed that the program was not conducting school age progress reports as required by EEC.
On the day of the visit the refrigerator used to store snack foods had dust and dirt in the inside.

The program did not have a current snack menu on the day of the visit.
Food is nutritious, adequate amount, with variety/meets USDA requirements
Licensor observed Potatoes Chips, Cheetos and other snacks, including Sunny D, that are not healthy options for children.
Through interview it was learned that the program was not offering an opportunity to conference with Educators regarding children’s development.

The program did not have a staff schedule posted on the day of the visit.
Inspection of the program's playground revealed that there was an insufficient depth of impact absorbing material under and round the fall zones for swings and climbers. There were exposed tree roots and landscaping fabric that could pose a tripping hazard. There was a large cement footing near a tree that could pose a danger for children. There was also concerned with large open areas in the fence that allowed children access to the street.
The evacuation procedure was not posted near the main entrance on the day of the visit.
The IHCP for a child with a chronic medical condition did not include the names of educators that received training addressing the medical condition or who trained them. Another child with a chronic medical condition did not have an IHCP on file.
Administration
Review of the Documents Library revealed that the program did not have a qualified School age Administrator. The Site Coordinator is missing 2 months of Administrative experience in order to qualify to be the School Age Administrator.
This renewal - monitoring visit inspection recorded no violations or advisories.
During the inspection of the outside play space it was learned that the following items are a potential hazards: (1) The swings did not have appropriate fall zone or an adequate amount of impact absorbing materials'; The chains holding the seat on the swings were rusty and the "S" hooks were not closed , (2).The arch climber did not have the appropriate fall zone and there was not an adequate amount of ground covering. (3) The large climber did not have an adequate amount of ground covering. (4) The zip line had rust and did not have an adequate amount of ground covering.
On the day of the visit,the current staff did not have current CPR training. CPR training must be renewed annually.

During the review of Individual Health Care Plans (IHCP), it was learned that not all children with identified chronic health care issues, had completed IHCP at the program.
