Child's file did not have written parental consent for the administration of medication
The prescription label on a medication for a child, required by an IHCP had an expiration 10/29/24 and had a discard after date of 10/27/25.

Review of the program's evacuation dill log revealed that the program had not documented a drill since 10/6/23.

Review of fire drill documentation revealed that the program did not conduct emergency evacuation drills in September and October of 2022.
A review of medical information revealed that one Individual Health Care Plan for a child with a chronic medical condition did not include parent signature. Another child’s plan did not include the names staff trained or who trained them.
