During the visit, the 4 records of the children present were reviewed. The following was observed : 1) Child #3 emergency contact information was incomplete in the enrollment packet. Additionally child did not have immunizations records in the file. 2) Children #2 emergency card (pg.#7) was incomplete in the file. 3) Children #4 did not have written consent for Certified assistant in the emergency card (pg.#7) in the enrollment packet. Additionally child did, child did not have immunizations records and lead screening results in the file.

EEC regulations states that garbage must be kept in lined and covered containers. During the visit, the trash can inside the bathroom was not lined and covered.
