During the follow up visit, the following was observed after reviewing the children's attendance log: 1) The weekly attendance log provided by the educator did not document the dates corresponding to the current week. Furthermore, the educator was observed documenting the dates during the visit. 2) The children's arrival and departure times were not accurately documented: - The attendance log did not show the correct departure times for the school-aged child on Monday morning after leaving for school, nor their return times from school to the program on Monday and yesterday in the afternoon. - The attendance records for the five children present today already showed their departures times documented for today 3/11/26.

During the visit, a child was inside an unlicensed bedroom upon Licensor arrival's. Educator had 3 separate exits (back through the kitchen, middle through the play area, and front). However , during the visit, the route of the back exit was obstructed and the following hazards were accessible to children: stove with a microwave on top, dresser, large plastic bins, box with empty bottles of beers, kayak leaning against the wall, bike on the ground in the pathway, piece of fence on the ground which educator said was used as a gate for the yard.

Educator did not have evidence of a well-balanced curriculum of specific, planned learning experiences that support the social, emotional, physical, intellectual and language development of all children.
