During the visit, 2 records of the children present and unrelated to the educator were reviewed, the following was observed: 1) Children #1 and #2 emergency cards (pg.#7 in the enrollment packet) were incomplete, and the Certified assistants did not have written consent. 2) Child #1: developmental history was incomplete in the enrollment packet. Additionally. the CPR/FA/medical consent (pg.#6) in the enrollment packet was incomplete and the Certified assistants did not have written consent. 3) Child #2: schedules of days/times in care was incomplete in the enrollment packet. Additionally, the child did not have completed the doctors information, and the CPR/FA/medical in the enrollment packet and the Certified assistants did not have written consent.

Last drill documented: 12-24-2025. After reviewing the evacuation log, the following was observed: 1) EEC regulations required to alternate emergency exits monthly during practice drills. At this time educator has 3 exits (front, middle exit leading to the driveway, and back exit leading to the yard). The evacuation log showed that the back exit leading to the yard was only used once between practice drills completed from April to December 2025. The front exit was used consecutive for practice drills completed in the months of October and November 2025. Educator did not alternate emergency exits as required. 2) The effectiveness (process) of practice drills was not documented for drills completed from April to December 2025.

During the visit, the children present were observed playing. Then, children transitioned to clean up and then to lunch time. Per educator the program was focusing this week on "Winter" theme. However, educator did not have evidence of a well-balanced curriculum of specific, planned learning experiences that support the social, emotional, physical, intellectual and language development of all children.
