Last drill documented : 10-7-2025. After reviewing the evacuation log the following was observed : 1) EEC regulations regulations required to alternate emergency exits monthly. Educator has 2 separate exits (front and back) . The evacuation log showed that only the back exit ("por la atra and bock") was used for drills completed from January to October 2025. 2) The evacuation log did not have EFFECTIVENESS of drills documented from January to October 2025.

During the visit, 4 records, including 3 of the children present wee reviewed. The following was observed: 1) CHILD #2: acknowledgement of parent handbook; parental visit notice; and doctor's information were incomplete in the recent annual update forms/enrollment. Additionally, the child's lead screening results were not up to date in the file. 2) CHILD #3: did not have general permission; and the CPR/FA/Medical consent was incomplete in the enrollment packet. 3) CHILD #4: did not have date of admission; schedule of days and times in care in the enrollment packet. Additionally, the doctor's information, general permission, and CPR/FA/Medical consent were incomplete in the enrollment packet. In addition, child did not have medical information (physical examination, immunizations records, and lead screening results) in the file.

During the visit, the children's table located in the kitchen area was not cleaned between uses, as required, after the children had eaten a snack (Cookies and juice) and then transitioned to a coloring activity at the table. Additionally, during the course of the visit, the educator was observed cleaning the same table with a small wet towel prior to lunch time instead of using and appropriate product to clean and sanitized the table as require to prevent and control infectious diseases.
