Four staff files were selected to review, and the following was found: 1) One staff member did not have evidence of a current PQ Registry. 2) Three staff members did not have evidence of a completed EEC Essential Training. 3) Two staff members did not have evidence of a completed orientation. 4) Four staff members did not have evidence of bi-monthly observations.
At the time of the visit, an Educator in the Toddler B Classroom did not wash their hands after coming into contact with bodily fluids.
During the Safety inspection, the following was found: 1) In Multiple Classrooms, chipping and peeling paint was found. 2) The bookshelf in the Preschool 3 Classroom was not secured.
During the IHCP review, the following was found: 1) One child did not have evidence of required medication in the Infant B classroom.

Three children's records were randomly selected for review, and the following was learned: 1) Two Child needed evidence of a current physical exam.
In the Infant B Classroom refrigerator, One child's cup and one child's food were not labeled with the name of the child for whom they were intended.
During the safety inspection of the program, the following was found: 1) In the Twaddler B Classroom, the dramatic play refrigerator and freezer doors were not able to open, and it was missing the plastic sink, causing an entrapment hazard. 2) Peeling and exposed drywall was found under the window sill next to the refrigerator in Twaddler B Classroom. 3) The corner of the top of the wooden sensory table was splintering in the Toddler A. 4) In the Preschool 3B Classroom the toilet paper was stored in an unsanitary manner when it was stored on top of the dispenser. 5) There were gaps of missing paint behind the sensory table in the Preschool 3B Classroom. 6) In multiple classrooms throughout the program, rust was found on the bottom of the doors. 7) Purple paint was chipping, and the bookshelf was not secured in the Preschool 1 Classroom. 8) The Kitchen set was not secured in the Prepper B Classroom.

An Educator in the Infant B Classroom was found sitting on the floor rocking a 9-month-old infant with a blanket covering the Infant's face.

Three staff records were randomly selected for review, and the following was learned: 1) One Staff needed evidence of 2 doses of[REDACTED] as required by regulation. 2) One Staff member did not have verification of two references.
Four children's records were randomly selected for review, and the following was learned 1) One child needed evidence of a current progress report.
The attendance in the Twaddler A classroom was not accurate. There were eight children signed in, and only nine were present.
An Educator in the Infant B classroom did not keep at least one hand on a child at all times when the child was being changed on an elevated surface.

In Twaddler B, three of the seven children's water bottles were not labeled at the time of the visit.
Nine new staff records were selected for review, and the following was learned: 1) Five staff members did not have evidence of a current physical, and one did not have evidence of a physical examination within one year of their hire date as required by regulation. 2)Four staff members did not have evidence of [REDACTED]. 3) Five staff members did not have evidence of an EEC Essentials Training completion certificate. 4) Five staff members did not have evidence of verification of two references as required. 5) Seven staff members did not have evidence of a completed program orientation.

During health and safety inspection of indoor space the following was found: not all classrooms are being maintained in a clean and sanitary condition, a clogged sink in infant room, material on multiple children's chairs is peeling off, diaper changing mats have worn areas that cannot be properly cleaned/disinfected, educators had personal drinks and bags/purses within reach of children posing potential hazards, rusted bathroom railings, caulking around some sinks areas are dirty and coming off.

Due to shrinking crib sheets, crib mattresses are curling up on edges. Mattresses should be firm and should maintain their shape even when the fitted sheet designated for that model is used.
A toddler educator was observed speaking to a toddler inappropriately when trying to get the toddler to put their shoe on. She stated, "I am not dealing with this today, do I need to call mommy?". The educator was not positively interacting with the child.
An [REDACTED]was found expired.
A teacher was observed putting a child down after diaper changing and not washing their hands until they cleaned diaper table and after the child had begun playing with toys.
No IHCP on file for a child with a chronic medical condition. One child's IHCP was incomplete and vague with instructions. One child's IHCP expired in 5/2022.

The following was found during health and safety inspection: paint peeling on windowsills and walls, a loose metal bracket that was used to secure 2 shelves, a broken play kitchen, a broken preschool chair, a staff person's purse and coffee cup found in an unlocked cabinet and a staff person's iced coffee found on a low shelf all accessible to children.

Many classrooms displayed empty shelves with limited toys/materials. One classroom had a dramatic play kitchen with no play food/pans etc... One classroom had limited books with only a few on thrown on a shelf.
