Licensor observed the record for every child enrolled and the following was noted: Child #1- outdated schedule, missing lead screening Child #2- IHCP outdated. Based on physical exam child has a food allergy but uses no life saving med. Child #3- outdated lead screening Child #4- ok Child #5- outdated lead screening

Licensor observed bathroom trash can is covered but has no linen.

LIcensor observed the record for every child enrolled in the program and the following was noticed: Child #1- missing date of admission Child #2 and Child #3- missing date of admission and parent daytime phone number. Children have ASQ but no progress report. Child #4- has ASQ but no progress report. Child #5- has ASQ but no progress report. Child #6- missing date of admission and parents daytime phone number. Has ASQ but no progress reports. In addition, all children record were missing assistants information in permissions section.

Emergency plan in parent handbook is missing method of communicating with parents during emergency.
Although Licensor observed fire drills documented monthly, they are happening only during morning time.
First aid it is missing telephone numbers for emergency services;
Educator did not have regular assistants certificate posted.
5 children were observed having ASQ but no progress reports on file.
Educator has a regular assistant working with her. Although Educator has ilcense and CPR/FA in the cellphone, theres no medical information, trainings information.
LIcensor observed refrigerator temperature was 20*F at the time of the visit.
