All children need a current annual update form. Permissions sections were updated but annual update form is missing. All children missing physical description. Child #2- outdated lead screening.
Fire drills are happening only in the morning according to the evacuation log.
Per Educator, HHM has a firearm. Licensor did not observe the letter to parents informing about it.

During the visit, Licensor reviewed the 4 records of the children enrolled, the following was observed: 1. A child physical exam was a not up to date 2. A child was missing immunization records on file 3. Two children were missing {REDACTED} results on file.
During the visit, Licensor observed the educator was missing an Individual Health Care Plan for a child diagnosed with a Medical Condition.
During the visit, Licensor observed a single slide in the outdoor space which measured more than 18 inches in height (22 inches) and the educator did not have a Fallzone as required.
During the visit, Licensor observed the Allergy List that was posted was missing a child diagnosed with a medical condition.
During the visit, Licensor observed the educator did not have a fallzone as required for two outdoor play equipments with slides measuring above 18 inches (single red slide= 21 inches, and house with attached slide =26 inches).
