The educator has a ripped diapering changing mat.
1. The educator has multiple hazards in the backyard such as: Bags of mulch, multiple tools, large bikes, an uncovered chimney, broken tree trucks and branches, a metal hammock frame, wood panels with protuberant nails, large bricks, uncovered grass mower machine, trash bags with broken branches, a broken outdoor umbrella, and other hazards accessible to children in the backyard. 2. There is an uncovered heater pipe that becomes very hot accessible to children in the backyard.

1. The secondary exit has 4 empty large water bottles posing an obstruction of the exit way. 2. The educator has a gallon of home insect killer spray solution accessible to children at the secondary exit. 3. There are multiple items clutter at the secondary exit, such as wood metal, rake, shovel, etc., accessible to children. 4. There is an uncovered grill accessible to children at the secondary exit. 5. There are gallons of paint accessible to children at the primary entrance.
1. The educator had a small heater in the bathroom with the loose cord on the floor accessible to children. 2. The educator has an electrical water machine cord hanging low accessible to children in the daycare room. 3. The educator has an unlocked cabinet under the bathroom sink with accessible gallon of soap, a rolled up extension cord, and plastic trash bags. 4. The educator has an unlocked cabinet under the kitchen sink with a vacuum, tools, paint, trash bags and other items accessible to children.
The educator did not have evidence of age appropriate activities implemented or curriculum planning. The educator acknowledged that she does activities as she goes and not with a plan or purpose to achieve.

1. There was no refrigerator for children’s food installed in the basement. 2. This licensor was unable to test the refrigerator temperature because there was no refrigerator. Please submit temperature when refrigerator is installed. .
1. There were 3 unbolted cabinets in the daycare room. 2. There was an uninstalled large paper towel dispenses on top of the kitchen counter posing a potential falling hazard on children.
1. The educator was using a portable floor heater at the time of this visit. 2. The educator did not have means of heat installed in the basement.
The water temperature tested 129.4*F degrees exceeding the maximum of 120*F degrees allowed per regulation.
The educator has a broken fence panel against the fence posing a potential hazard for children. 2. The educator has an uncovered grill and clutter accessible to children. 3. There were cement bricks accessible to children on the basketball hoop base positing a potential hazard for children.

Although the educator is conducting monthly evacuation practices, she is not documenting the length of the practices.
The educator has an expired FA. FA expired 7/12/2021. CPR exp: 7/6/2022 online training. Discussed with the educator that she needs to send the licensor the certification letter stating that she went in-front of the instructor.
The educator is not using a disposable barrier for diapering.
1. The educator has a electric fan with loose cables hanging from the interior stairway accessible to children in the daycare room. 2. The educator had AC extension cord along the floor, accessible to children in the dining and living room.
The secondary exit steps was obstructed with a mop bucket with water and another bucket with items on the exit steps.
The educator did not have a Carbon Monoxide alarm in the program.
The educator did not have liquid soap and paper towels accessible to children for hand washing.
