The educator stated that she has not been using a method to ensure that children carry or have accessible the program's name and telephone number when leaving the premises to walk to the park. The licensor discussed regulation 7.11.(6)F. Each child must carry on his/her person the name, address and telephone number of the educator or childcare program whenever s/he is off the premises in the care of the program.
Parent Handbook
The licensor reviewed a sample packet of children records and observed the following: 1)Child #1 did not have admission date on page one of the enrollment packet. 2)Child #2 did not have admission date on page one of the enrollment packet. 3) Child #3 did not have admission date on page one of the enrollment packet.
At the time of the visit, the parent handbook was not accessible.

At the time of the visit, the licensor observed the following indoor/outdoor hazards: 1)A standing shelf in the daycare room that was not properly secured to the wall posing a tipping hazard. 2)A standing bookshelf that was not properly secured to the wall. 3)A walking pad that was leaning against the wall and poses a tipping hazard. 4) A bathroom cabinet that was unsecured that contained wipes cleaning products, and personal items accessible to children. 5)In the bathtub four different shampoos, and conditioners were accessible to children.

At the time of the visit, the licensor observed that the educator has not been alternating the exits and has been using the front entrance for every practice drill. As well as, the smoke detector log has not been filled out since March 20,2026.
There is a metal swing frame that is loose and the bars are uncovered posing a potential hazard for children.
Although the educator has some materials, the educator needs a bookshelf, manipulative toys, math, science, blocks, music toys.
The educator has 1 playpen and 1 mat. The educator needs 4 mats.
The educator did not have the written evacuation plan.
The first aid kit needs the CPR mouth guard and tweezers.
There are 2 unbolted shelves in the daycare room.
The water temperature tested 138.8*F degrees exceeding the maximum water temperature of 120*F degrees allowed per regulation.
The educator does not have a PQR.
