Licensors observed the educator documenting arrival times for today during the visit. 3. The educator used the attendance log for the week of 9/30/19 to 10/4/19 to document the attendance for today 10/24/19 for the two {REDACTED} children currently enrolled 4. The educator did not present evidence of certified assistant {REDACTED} attendance.

Licensor observed the following indoor hazards accessible to the children: 1.uncover outlet in the playroom 2.bucket with dirty water inside the bathroom tub 3.dirty mop in the bathroom 4.large stuffed animal that was ripped and a child was removing the stuffing material from the inside which represents a choking hazard 5.upon licencors arrival the educator yelled to the parent in the kitchen "hide all the knives that the state is here" in Spanish Note: although licensor inspected the kitchen unknown knives that the educator referred to were not found. Licensor observed the following outdoor hazards accessible to the children: 1.uncovered recycle bin fill with trash 2. orange vacuum with loose cord 3. four car tires 4. no gate at the bottom of outdoor stairs leading to second floor level Note: The educator stated that she is no using backyard lately due to EEE mosquitoes and parents not wanting her to take their child outdoors.
