The licensor reviewed 3 of the children records. The following were observed: 1.One of the children record did not list the educator’s name as part of the general permission to basic transportation and permission to transport to medical facility. 2.Three of the children records did not list where the child can go when off the premises (the educator is not approved to use yard). 2.Two of the children records did not list the child’s date/time of attendance with time in an out. 3.One of the children records did not have a picture or physical description of the child. 4.Three of the children records did not list “topical medication/ointments that child can use(see page 6 of enrollment).
