One file did not include documentation showing the completion of the 12 Essentials Trainings. Bi-Monthly observation of staff working with children were not available for review.
At the time of the visit, 28 children were observed lying on resting mats in the Preschool Classroom. The maximum group size for preschool is 20 children.
During the visit, a child was observed resting in the office area. This space is not age-appropriate for children. Child care must be provided only in spaces approved by the Department. When measuring activity space, only usable floor space (exclusive of hallways, bathrooms, and portions of rooms or areas that contain furniture or equipment suitable only for adult use) may be included.
A Monkey Bouncer was observed on the playground in an area that did not have impact absorbing material under and around the fall zones.
Four educator's purses were observed on a table in the Preschool Classroom and accessible to the children.
One child's Individual Health Care Plan expired on 11/8/2023 and did not include all of the required information. A second child's Individual Health Care Plan did not include all of the required information.

An electrical outlet in the Preschool classroom did not include a safety cover to prevent children from accessing the receptacle. A toilet brush and two plungers were located in the children's bathroom and accessible to the children. The garbage barrel in the Pre-K classroom was not covered.

The following hazards were observed on the playground. There is not an adequate depth of impact absorbing material under and around the fall zones of the brown climbing structure and swings. There was not an adequate depth of impact absorbing material under and around the fall zones of the seesaw and brown slide. The dome, seesaw, and brown slide did not have the required six feet of fall zone. The lattice covering the cellar window and around the rear deck area is broken. Brooms and paint roller handles stored next to the deck must be removed from the children's activity space.
