Physical B. Annual update (last time parent signed was 8/31/23). C. The general permission did not specify where the child can go when off the premises. D. The general permission to provide basic transportation and to transport to medical facility is not accurate when 3 educators are listed, but there should be 2 educators listed. No barrier at the bottom of the stairway at the lower level, leading to the first floor.
Licensor was not able to review training hours at the time of the visit. The educator reported that her training certificates for the current licensing cycle were not on site.
Licensor observed the following indoor safety hazards: 1. The trash container in the bathroom is not covered. 2. Two poles in the basement are not properly padded.

The educator reported that the total enrollment at this time is 12 children. She stated that she doesn't have more than 7-8 children at a time. According to the attendance record submitted by the educator for the month of May 2021, the educator provided childcare to 8 children, including {REDACTED} child and 7 children under the age of {REDACTED} on 5/5/2021 and 5/6/2021. On 5/5/2021 and 5/6/2021 the program exceeded its licensed capacity.
No evidence of progress reports for 4 children.
According to the attendance record submitted by the educator for the month of May 2021, the educator provided childcare to 8 children, including {REDACTED} and 7 children under the age of {REDACTED} on 5/5/2021 and 5/6/2021. Certified Assistant {REDACTED} was present on those days, but {REDACTED} FCC license expired on 10/6/2020. Certified Assistant {REDACTED} is not an EEC approved caregiver.
The educator is the only EEC approved caregiver staffing the program at this time. 1. The FCC license for Certified Assistant {REDACTED} expired on 10/6/2020. 2. The FCC license for Certified Assistant {REDACTED} expired on 11/6/2020 According to the attendance record submitted by the educator for the month of May 2021, Certified Assistant {REDACTED} worked with her on 5/3/2021, 5/5/2021, 5/6/2021, 5/12/2021 and 5/13/2021 without a valid FCC license.
This change of space visit inspection recorded no violations or advisories.
