There was a large air purifier and other items stored on top of the wooden cube, used as a small quiet space by children. The items count tip off the cube and pose a danger to children.

At the time of the visit the children’s record checklist was incomplete.
Review of Staff meeting documentation revealed that the program was not conducting two hours of staff meeting per month. There was no documentation of staff meetings for April, May, June, August and September 2019
