Indoor Space
There were two rugs that were curling up which pose a tripping risk to children. The bottom of the fridge was dirty with an old spill. Garbage cans did not have covers.
During the observation of the outdoor space there were several areas of concern: 1. The climber with the slide had weathered wood that could pose a risk to children, the slide had a piece of plastic that was missing, and the ladder rope (chain net climber) was very weather with material that was weathered. 2. There was a small wooden picnic table that was very weathered and poses a risk to children. 3. One of the spring rockers had exposed rusted metal on the spring area that could pose a risk to children. 4. The horizontal ladders had blocks under them, those need to be removed as it compromises the impact absorbing material. The height of the horizontal ladders needs to be checked as well children can't use them if they are over 60 inches.

It was observed that the side of the refrigerator was dirty and there was a ceiling tile falling in the bathroom. The front entrance had clutter.
The fall zone under and around climber with slide is not adequate.
One child has an IHCP but it’s not listed on the allergy list.
One IHCP was incomplete.
Some cereal was stored in a covered container but was not dated. Other cereal was left in unsealed bags.
It was observed that there was peeling paint on windowsills and the wall in the housekeeping area.
