Playground Safety - Fallzones
Emergency Evacuation
There was 1 school age staff who had an out of date physical in their record. Please review all staff records and ensure they are all complete and up to date.
In the preschool playground, mulch needs to be added to the fallzone as it is not currently an adequate depth. In the toddler playground, the plastic slide needs to be moved to the fallzone.
In the infant room, there was an exersaucer blocking access to one of the exits as it was right in front of it. There were also 2 trash bins turned sideways in front of the exit door, which blocked the emergency crib from being able to fit through the doorway.
In the preschool/teddy bear room, the children did not wash their hands before getting their lunch boxes and eating lunch. The teacher also did not wash their hands before helping the children to open their lunch boxes.

Food stored in clean/covered containers
Not all food was stored in a sealed container as required, creating a potential hazard.
During a review of the program, the following was observed: 1) Chipped paint in the reach of children throughout the ECE section of the program, creating a potential hazard 2) rest mats were ripped, with the foam exposed, creating a potential hazard 3) in the "tigers' classroom, the kickboard is missing from the wall and there is a large gap between the tile floor and the wall, creating a potential tripping hazard. This was addressed with the program during the 2024 visit, but it has not yet been repaired.

During an observation of a diaper change it was observed that the Educator did not use a "2 step" cleaning and disinfecting process as required by regulation. Additionally, the same educator was observed to not clean or disinfect the diaper table as required in between changing Child A and Child B, creating a potential risk to children.
No peeling or flaking paint (indoor and out)
During an observation of a diaper change in the toddler program it was observed that the educator did not keep their hand on a child while changing them on an elevated surface as required, creating a potential danger.
During an observation of the indoor space, the following was learned: 1) stained ceilings were observed throughout the building, creating a potential risk to children. 2) The "kickboard" in the upstairs "snoopy" room was observed to be coming off of the wall, creating a risk to children 3) in the "Tigers" room it was observed that the floor is not flush with the wall, creating a gap and potential hazard to children 4) The "kickboard" is pulling away from the wall in the "Tigers" room, creating a potential danger.

6 staff records were randomly selected for review and the following was learned: 1) Educator A was being used in a "group leader" capacity but did not have documented written evidence to support that they had the qualifications to be used in that role as required.
During a review of the "kitchenette" space it was observed that not all food was stored in a sealed container as required by regulation.
During a health and safety walkthrough of the program it was observed that there was chipping paint in several classrooms, creating a potential hazard to children.
During an observation of a diaper change, the educator was observed to not wash their hands or the child's hands after diapering as required.

Some rest mats were observed to be torn, with the foam exposed to children, creating a potential hazard.
On the day of the visit, the following was learned: 1) the program did not have the appropriate depth of the impact absorbing material surrounding all equipment as required by regulation. 2) On the toddler playground, the screen from the bottom of the playground surface is pulling up, exposed to children. Additionally, it shows that that playground does not have the appriopriate impact absorbing material surrounding the equipment as require.

Diapering and Toileting for children under 2.9
5 staff records were randomly selected for review and the following was learned: 1) A staff member who started on 10/2/2022 did not have evidence of having completed their "Strong Start" trainings as required 2) 1 staff member did not have evidence of bi-monthly staff observations as required.
6 children's records were randomly selected for review and the following was learned: 1) 2 children did not have evidence of a current face sheet, first aid/emergency consent from, off site consent, on site swim consent, transportation plan or developmental history form as required.
1) The light cover in the "community" school aged space was observed to be cracked in half, creating a potential hazard to children. 2) Stained ceiling tiles were observed in the main hallway space and in the infant room, creating a potential hazard to children. 3) in the "tigers' classroom, it was observed that there is a hole in the door frame at one of the emergency exits, creating a potential hazard to children.
Not all exit doors had evacuation procedures posted as required.
During a review of 2 separate diaper changes in two different toddler rooms, the following was observed: 1) In the "kolas" room, the Educator was observed to change the paper barrier between children, but did not wash, then disinfect the surface between each diaper change as required. 2) The same educator was observed to change their gloves between diaper changes but did not wash their hands as required between diaper changes. 3) In the "pandas" room, the educator was observed twice during changes of two different children, to take their hands off the child while changing on a evaluated surface 4) The same educator was not observed to wash their child's hands after changing their diaper as required. The same child was then observed to sit down at the snack table for snack, but never washed their hands.

During a review of the school aged program, it was observed that children got off the school bus, then walked into the gym space. During that time, children were encouraged to have snack but were not observed to wash their hands prior to eating.
It was observed throughout the Early Childhood classroom space, that the tile floor has holes in the flooring, creating a potential hazard to children. In the "snoopy" classroom (located upstairs) it was observed that on the cubby shelf to the right as you first walk in, the "kickboard" is pulling up, accessible to children. In the Infant room, several stained ceiling tiles were observed, creating a potential risk to children.

5 staff records were randomly selected for review and the following was noted: One staff did not have evidence of completing the "strong start" online trainings as required. One staff did not have evidence of a current physical on record.
It was observed in several classrooms, as well as outside on the "red bench" that paint was chipping, exposing to children, creating a potential hazard.
During a review of the playground space, the following was observed: the impact absorbing material was measured and did not meet the EEC requirement of 9 inches around the fall zones. The basket ball court (accessed by the school aged children for outdoor play) was observed to have a section where the surfacing has "pulled up" exposing the pavement under it. The ground was also observed to be uneven due to the surfacing pulling up and the cracks that have formed in the surfacing as a result.
It was observed that not all classroom doors had evacuation procedures and maps posted as required.

Sections of the "kickboard" were observed to be pulling off the wall in the school aged space. Stained ceiling tiles were observed throughout the program. Adult scissors and the educators iced coffee were left in the reach of children in the school aged space.
7 children's records were randomly reviewed and the following was learned: although Child A had evidence of a completed developmental history, it was not signed and dated by their parent as required. One child did not have evidence of a current physical on record as required. One child working with EI, did not have evidence of a progress report every three months by the educator assigned as that child's liaison as required by regulation. One child did not have evidence of a current progress report as required.
An educator's lunch bag was observed to be stored in the reach of children in the "Pre-K" room, however the Licensor was able to observe a [REDACTED] pill bottle stored in that lunch bag, creating a potential danger to children.
The Basketball court (where SA play) was observed to be chipping in several areas on the court, creating an even surface, creating a potential tripping hazard. There were sections of the black fence that surround the basketball court that have been pushed in, creating a potential entrapment hazard. The basketball court was observed to have trash thrown on the ground throughout the court, school aged staff were observed to be on the court with children, but no one was observed cleaning up the trash so children did not have access to it. This showed a lack of judgment on behalf of the school aged staff. During the visit it was observed that 3 preschool "emergency backpacks" were left in the reach of children when they were hung over the fence inside the playground.

5 staff records were randomly selected for review and the following was learned: 2 staff did not have evidence of a current PQ registry, 1 staff (who was hired on [REDACTED] did not have evidence of a physical on record, 2 staff did not have evidence of current MMR records on file, one staff did not have evidence of a second MMR on record. One staff did not have evidence of observations conducted every other month as required.
Not all equipment was observed to be secured as required in the early childhood program. It was observed on the "Preschool" side of the early childhood program that the floor tiles are pulling up, chipping and are cracked in many sections, creating a potential hazard to children.
Chipping paint was observed in the School aged/Wide world of tots classroom.
Not all doorways were observed to have evacuation instructions or maps as required.
A child that required an IHCP plan did not evidence of a current plan signed by the doctor.
