The changing pad in infants was observed to be torn in two places. A package of sealed diapers was observed to be stored directly on the changing table in toddlers. Stained ceiling tiles were observed in toddlers and the “upstairs” School aged space. Chipped paint was observed in the toddler room, near the book area and near the kidney table. Paint was also observed to be chipped on the top of the divider between the two rooms (allowing chipped paint to fall on the ground and potentially in the reach of children).
