Pole in the basement has a wooden base with sharp corners that does not have bumpers on all 4 sides. Large set of wooden cubicles are not secured to the wall and swayed when touched. Gates were not in place at the bottom of the basement stairs and the stairs in the living room leading to the second floor. Gates must be in place at all times whenever children under the age of 3 years are present.

When the licensor arrived a 9 month old infant was observed strapped in and asleep in a car seat placed on the playroom floor. The licensor asked the educator to move the child into a nearby pack and play. Safe sleep practices were discussed with the educator. The educator must sign and return the safe sleep pledge to the licensor. Also, the parents of the infant must sign the Safe Sleep Parent letter which must also be returned to the licensor when completed. The licensor will also conduct a safe sleep follow up visit to the home within 10 business days.
