Upon arrival to the FCC home, licensor observed an infant sleeping on the educator's bed on top of a Boppy nursing pillow. The infants body was facing down with his face sideways on the pillow, and there was a blanket placed on top of his body. The infant was also wearing a bib. We discussed the regulations, and the educator removed the infant from the bed and placed him in a pack n play. The educator stated that the parent requested for the child to sleep in this manner. A copy of the Safe Sleep policy and pledge has been sent by email for the educator to review and complete and notify parent by providing a copy of the Appendix B: SAFE SLEEP PARENT LETTER, FIRST CITATION; SAFE SLEEP PARENT LETTER - First Citation of Non-Compliance Findings (Provider).
