At the time of the visit, the following was observed: within the Toddler One classroom there were six students with one educator (as the co-teacher took some students to the bathroom located outside of the classroom). Later, it was observed that a teacher took one student to the bathroom, leaving an educator alone with eight students in the Toddler One classroom. In the infant classroom it was observed that one educator stepped out of the classroom, leaving the co-teacher alone with five students. The program must have the number of educators necessary to ensure adequate supervision of the group at all times.

At the time of the visit, it was observed that a seven month old, within the Infant Classroom, was fed a bottle while in a 'baby bouncer' seat. At the time of the visit, it was identified by EEC that educators need to hold infants while feeding. Once identified, the educator in the room held the student and continued to provider them with their bottle. In programs serving infants and toddlers: Infants must be held while fed a bottle.

At the time of the visit, the outdoor area was observed to have turf, it was unable to be determined if there was a proper fall zone under the turf. It was observed that the toddler play area and the preschool play area both had slides that did not have an adequate use zone of six feet in all directions. The use zone should extend a minimum of 6 feet in all directions from the perimeter of the standalone climber.

At the time of the visit, the following was observed: within the Toddler One classroom two students allergies were posted on magnets, uncovered and visible. Within the infant classroom, a list of student allergies was posted on the wall, uncovered and visible. A list of allergies and/or other emergency medical information provided by the parent for each child and a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong must be in a manner that protects the privacy of each child.

At the time of the visit, in a review of the Emergency Evacuation log, the documentation was not fully complete. Documentation of August and September 2025 drills were missing. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

At the time of the visit, in a review of of two students IHCPs, the documentation was not fully complete. Student C had an EEC IHCP but was missing a doctor provided IHCP. Student D was missing documentation of 'potential consequences if medication is not administered'. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
