Reviewed children’s files. It was observed that files had enrollment packets from the agency (child development) and EEC enrollment packets within the same file. It was observed that one or both packets had missing information, such as date of admission, missing parent signatures and signature dates, transportation section not completed, acknowledgment for receiving parent handbook not signed and dated, missing emergency cards, and one file needs to be updated with current signature dates. It was advised to the educator that she must ensure enrollment packets from Child Development are complete and are signed. The educator will need to complete children’s records checklist to help with having complete enrollment packets. One file needs an updated physical and two files need lead screening results.
