Within Preschool One there was a closed trash bag by the handwashing sink. An educator in the classroom identified it had discarded sensory table sand in it and the bag had been there for several days. By the circle rug the baseboard had been removed. There was a dehumidifier plugged in with the cord loose and accessible. Within the Preschool Three classroom there was an Alexa device plugged in, where the cord was loose and accessible to students. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning. At the time of the visit, the following was observed outdoors: The ramp leading to the playground had a handrail with slats. The slats were 4.5 inches apart and a potential entrapment hazard. On the preschool playground, The wooden fence was coming loose, which resulted in three nails protruding from the fence. The handrails on the preschool play structure had chipping and peeling paint, resulting in the metal structure being exposed. There was a steering wheel on the play structure that was broken, and hanging off of the play structure. A blue slide on the play structure had two cracks in it at the top, which could potentially pinch a child as they sat down. There was a play kitchen that was unsecured and tippable. On the toddler playground there was a play tool bench which was unsecured and tippable. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children.

The floor in the classroom appeared buckled and there was chipped and cracked tile. -In the Pre-K classroom, the lego table was uncleaned. There is evidence of water damage on the ceiling near the fire exit sign and door. There is cracked tile in the threshold of the classroom door. The blue child-sized couch has rips on it. The trash was full and overflowing. The carpet in the classroom had rips and frayed edging where two different type of carpets meet. -In the Preschool 1 classroom, there was a small hole in the wall. The sink, counter area around the sink, and mirror were dirty. The white cabinets under the sink are in disrepair and are unclean. -In the Preschool 2 classroom, the circle area rug was dirty. The wall area near the circle rug was dirty. The door had peeling paint on it. -The bathroom area stalls have broken toilet paper dispensers, stains on the floors, and the tiles appear to be cracking. The wooden step-stools in the bathroom are unclean. *The following non-compliances were cited in previous visits to the program as follows: -Visit on 6/28/19: carpet in Infant room appeared dirty. Plan for Correction submitted by the Provider on 7/1/19 stated that a request for a new carpet was made. -Visit on 12/5/18: Throughout the facility, classrooms and equipment are dirty and in need of cleaning; chipped and peeling paint in classrooms; rust on children's chairs; blue rug dirty; holes in wall. The Plan for Correction submitted by Provider stated that the interior of the center was to be painted by 1/2019 and that old and dirty equipment had been removed and new equipment ordered. -Visit on 10/29/18: Throughout the facility, classrooms were dirty, with visible grime; rusted chairs in Preschool, holes in the wall, rug dirty. The Plan for Correction submitted by the Provider stated the chairs and rug were replaced as of 4/20/19. -Visit on 1/20/11: white cabinets under sink in Preschool were in need of cleaning; chipping paint. -Visit on 8/6/08: chipping paint and floor buckling were cited.

The following hazards were observed and accessible to children during the visit: -In the Infant class, diaper cream, plastic bags, and Glade air freshener were accessible to children under the diaper changing table. A broom was accessible to children. -In the Toddler class, diaper cream, plastic bags, and a container of small art supply jewels were accessible to children under the diaper changing table. There was a box of sippy cups with water left over in them from the summer, staff stated, and were accessible to children under a cabinet. The metal brackets connecting the shelving units and the play kitchen area were protruding out and the screws were either loose or on the floor nearby. -In the Toddler 2 class, there was a broom and dust pan accessible to children. There was soap, used sponges/scrub brushes and diaper cream accessible to children under the diaper changing unit. -In the Pre-K class, a refrigerator wire longer than 6 inches was accessible to children. An ABA therapist was in the class and had her purse unattended on the floor, accessible to children. -In the Preschool 1 class, there was a box of laminating supplies and wires accessible to children on a low shelf. A broom was accessible to children. There was an uncovered electrical outlet near the rug area. -In the Preschool 2 class, a roll of plastic trash bags were accessible to children on a low shelf. There was a pair of adult scissors in the front pocket in the First Aid Kit bag accessible to children. There was a pest repellent plugged into an outlet in the classroom. -The adult bathroom was unlocked and accessible to children and it contained a wheeled cart full of paint supplies and sealant. -The hallway near the Director's office is accessible to children and contains a copy machine with long wires, a computer with wires, a vacuum, and center emergency bags at a child's level. -The front desk in the hallway had a computer with long wires as well as staples, and rubber bands in it. -Two supply room doors were unlocked and open. One contained a toaster oven, microwave, and laminating supplies. Another supply room had multiple cleaners, trash bag liners, paint, and mop and bucket. -There were two staff bags in the hallway (leading to the playground) under a table. *The same non-compliances were cited on the following visits: -10/29/18: Teacher purses accessible to children; numerous toxic substances were accessible to children.
