At the time of the visit, the following was observed: within the Orange Classroom the play kitchen was not secure. Within the Green Classroom the play kitchen and a book shelf were not secure. Within the Toddler Playground area there was a play kitchen that was not secure. All unsecured items were tippable. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed and non-tippable. Within the Infant playground there was a climbing play structure that did not have a use zone of six feet in all directions. The use zone should extend a minimum of 6 feet in all directions from the perimeter of the stand alone climber.

At the time of the visit, it was observed that the Orange classroom attached bathroom had a trash can that was not covered. The trash was used to dispose of soiled diapers/ pull-ups. Garbage must be kept in lined and covered containers and all trash containers must be emptied at least daily.

At the time of the visit it was observed that the staff schedule was not posted. The Director identified the schedule was taken down due to the program being repainted. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.

At the time of the visit it was observed that the doors within the hallways, leading directly outdoors, did not have evacuation procedures posted. At the time of the visit, the Director posted evacuation procedures at two of the doors. Evacuation procedures must be posted at each exit; Exit signs must be posted in rooms with direct access to outdoors; Diagrams of escape routes must be posted in rooms without direct access to outdoors.

At the time of the visit in a review of three students IHCPs and the corresponding prescribed medications, some medications were not readily available. One student did not have their prescribed antihistamine on site. Another student had their medication stored in a locked medication box in the hallway. Two students were missing documentation of the educators trained on their health condition. One student was missing documentation of the symptoms of their health condition. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

One student had expired medication consent. One student was missing documentation on if they had taken their medication previously and another student was missing directions for storage. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.
