The following hazards were observed in the outdoor playspace, accessible to the children: - Gaps at the bottom of the wire fence, posing an entrapment hazard and allowing access to the neighbor's yard - An uncovered grill - A propane tank - Various plant pots filled with rainwater - In the back of the building, a section of the black fence did not meet the required 4-foot height and had gaps at the bottom, providing access to the busy street - The black gate in the back of the building had a large gap at the bottom, which could also allow access to the busy street - Branches were protruding through the fence and posing a safety hazard.
