The programs self created medication consent document did not include the dates for the medication to be provided. In a review of nine students files, all nine had developmental history information that was not completed and signed on a yearly basis from the parents. Administration reported that the information was reviewed yearly with parents, however this was not documented. Enrolled families must complete a Developmental History for their child on a yearly basis. Three students had transportation plans that were completed but did not have the date of completion indicated. One student had an incomplete transportation plan.
