During the visit, the large playground area was observed to have multiple hazards. The following was observed on the playground: There was unknown vegetation growing through the fence, into the play area and accessible to students. There were many different holes of various sizes within the pour N' play ground surfacing. One of the slides had a large crack/ hole in the base of it, creating a hazard for anyone who would potentially use the slide. The outdoor play area must be free from hazards. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate. At the time of the visit, the program did not use the playground area.
